Hours/Schedule: 20 hours/week, flexible schedule
Rate:  $25-$30/hour plus commission

Job Description: Responsible for all sales and customer service activities from lead generation through close. Becomes the leading home care expert and transitional care specialist in our community.  Develops and implements the Sales and Business Plan which will meet both the personal and business goals of expanding market share. Works with the sales and administrative staff to achieve service excellence and create world-class experiences for our patients, families, and physicians.

  • Responsible for the sales and marketing of home health services in the territory
  • Educate the referral community regarding the value that we provide specifically in terms of patient satisfaction, patient outcomes, transitional and case management techniques
  • Demonstrates technical selling skills and service knowledge that allows for effective formal presentation
  • Present timely patient outcome reports to physicians and other members of the referral community. Demonstrate that patients get better while under our care. Add email addresses to our newsletter list
  • Perform discharge planning services. Inform hospital/snf referral sources that as part of our transitional care model, you would like to meet all patients prior to their discharge home. We want to play an active role in ensuring a smooth transition from hospital to home and gather all relevant information needed to ensure patient transitions home are successful and avoidable hospitalizations are avoided altogether
  • Communicate regularly with caregivers and care team members that care for patients in the assigned territory.  Encourage/initiate daily the sharing of information so we are all well informed and can speak to physicians about relevant and timely matters related to their patients
  • Invest in Networking Get input from your key management staff, and identify contacts within each of your current and potential referral groups. Identify all community and state associations and determine the value of attending meetings and workshops
  • Uses the CRM/database to organize and manage relationships, referrals sources and marketing activity
  • Participates and contributes to the development of educational programs offered to the community, referral sources, and employees
  • Builds trust, values others, communicates effectively, focuses on the customer, solves problems creatively and demonstrates high integrity

  • 2-5 years in the home health industry or other related sales position
  • Strong understanding of customer and market dynamics and requirements
  • Willing to travel and work in an assigned territory working with a wide array of healthcare professionals
  • A positive attitude and willingness to listen, educate and represent the brand well
